Ismael Diaz, M.D.
Ismael Diaz, M.D. graduated with honors from University Of Texas Medical School At Houston in 1998. Dr. Diaz has more than 19 years of diverse experience, first as the owner of Diaz Family Medicine Clinic in Atascocita and then joining Wellspire Medical Group in 2013. He has a passion for working with patients ranging from pediatrics to geriatrics.
Dr. Diaz is well-known and respected in our community and is a member of the American Medical Association, the American Academy of Family Physicians and the Texas Academy of Family Physicians.
In Dr. Diaz’s free time he enjoys exercising and spending time with his wife and three kids.

Yanet Delgado
Yanet Perez Delgado, FNP-C, graduated with honors from South University, Savannah, Georgia in 2022. Before that, she graduated from the Higher Institute of Medical Sciences in Havana with a doctorate in 1996, then completed 4 years of post-grad specializing in Anesthesiology in 2000.
She brings to our team over 20 years of experience in the medical field. She is a member of the America Association of Nurse Practitioners, is passionate and committed to high quality care in the interventional medicine she does. She loves the sea and open spaces, in her free time she enjoys exercising and reading.
